Social media is a prominent aspect of people’s everyday life but remains a potentially dangerous experience for people with photosensitive epilepsy, a condition in which exposure to certain visual triggers may induce seizures and other adverse effects. To explore alternatives for existing rule-based algorithms for detecting visual triggers in media, we conducted a preliminary interview study with 4 participants with photosensitive epilepsy and an initial experiment with 49 participants to determine the feasibility of crowdsourcing in the detection of content that may trigger seizures. We compared the crowdsourced responses with the labels of PhotosensitivityPal (a tool with high accuracy in detecting and classifying visual triggers), which indicated that crowdsourcing seizure trigger risk identification was overall feasible. We provide design opportunities for human-powered accessibility, both in the implementation of tools and in the improvement of crowdsourced detection.

OBJECTIVES
The purpose of this study was to qualitatively examine preconception health and counseling experiences of people with mobility disabilities through evaluation of personal lived and professional experiences.
METHODS
From March 2023 to July 2023, we conducted a descriptive qualitative study of free-text responses collected as part of a larger Delphi study involving a convened panel of experts, including physicians, researchers, and individuals with lived experience of mobility disability, in preconception care and education for individuals with mobility disabilities. Participants were identified by a systematic review of published and gray literature and purposive sampling. Free -text responses were collected from waves of the Delphi study survey questions and analyzed using constant comparison analysis to develop thematic results from the raw data through comparison of experiences, beliefs, and concepts noted within and between respondents.
RESULTS
The participants (n = 17) in this study were a group of experts on preconception health and disability, including individuals living with mobility disabilities (n = 7) as well as physicians and researchers in fields related to pregnancy and disability (n = 10). Four major themes were identified: strong patient interest in preconception counseling, ableist perceptions strongly affect the presence and quality of preconception counseling, implicit barriers to preconception counseling and pregnancy planning, and expectations and concerns about a multidisciplinary approach to preconception counseling.
CONCLUSIONS
Personalized preconception health is of great interest to women with disabilities, and there are clear knowledge and communication barriers to optimal evaluations of preconception risk and interventions to improve health before and during pregnancy.
PRACTICE IMPLICATIONS
Preconception education should be personalized to the specific medical needs and social challenges present for people with mobility disabilities considering pregnancy.

Vaidya: None. N.P. Krone: None. Background: Congenital adrenal hyperplasia (CAH) is one of the commonest forms of primary adrenal insufficiency with an incidence of about 1 in 15,000. Previous studies have highlighted the suboptimal health status and care provision in adults with CAH and these were associated with significant co-morbidities. In 2023, we implemented CaHASE2 (https://www.endocrinology.org/clinical-practice/research-projects/cahase-2/) to develop a strategy for prospective collection of longitudinal data. Our recent CAH service evaluation suggested significant differences in the approach to CAH patients. Aim: To identify specific unmet needs in the care of people living with CAH, through standardised phenotyping across all participating centres. Methods: In September 2023, PIs agreed a minimal dataset for the collection of real-world data for participating centres. The data is collected using the international CAH registry (I-CAH; https://sdmregistries.org/). CaHASE2 was launched in November 2023. Results: To date, 351 adults (213 females, 138 males) with CAH have been recruited and 1213 clinic visits were available for analysis. There is a preponderance of younger to middle-aged adults in the currently available datasets (median age 42 years, range 23-88). Preliminary analysis suggests a temporal change in glucocorticoid choice over time with an increased use of hydrocortisone and a decreased use of prednisolone. Analysis of 17OHP concentrations shows that a significant proportion of patients are overtreated. A significant proportion of patients are overweight or obese. Currently 18 centres are actively recruiting and 5 are awaiting local approval to use the I-CAH registry. The data will be analysed in 12-month cycles, to assess the current level of care provision and inform the development of CAH standards. In addition, we will establish a report that will provide centres with information about their local care provision in relation to other centres. Conclusions: The CaHASE2 project will provide important information about the health status of adults with CAH and how this might be related to differences in health care provision. Background
Acetabular cup migration is a rare complication of hip replacement surgery. If the acetabular socket of the ilium is weak, the prosthesis can erode or pass through it. There have been multiple reports of acetabular cup prosthesis migration to the colon, creating a coloarticular fistula between the hip joint and the cecum, sigmoid colon, or rectum. We report a rare presentation of a complete intracolonic migration of a prosthetic acetabular cup without creating a fistula.
Case Presentation
A 53-year-old female patient with a remote history of a total right hip replacement presented with abdominal pain and rectal bleeding. Computed tomography imaging revealed that the acetabular cup prosthesis was inside the colon. The patient underwent a colonoscopy as well as exploratory abdominal surgery, and the object was retrieved. The patient recovered without complications from the surgery. The acetabular cup migrated into the colon without formation of a residual fistula or any notable complications involving the hip joint despite the loss of the acetabular component.
Conclusion
The mechanism is unknown for how the acetabular prosthesis was able to pass from the hip joint into the colon, without a fistula, bowel perforation, or significant hip injury. At the time of writing, this case appears to be the first to document a complete intracolonic migration of a hip prosthesis.

The development of continuous-release devices or injectables for the long-term delivery of biologics is of great interest, especially monoclonal antibodies (mAbs) that require frequent, high-dose injections. Preclinical testing of these technologies in murine models is necessary for clinical translation; however, xenogeneic responses to the mAb and foreign body responses to the implants or injectables can confound results. Immune system knockout (KO) models that affect immune cells are often used in these experiments, but the effects of KO models on mAb pharmacokinetics (PK) are not well characterized. Here, we investigated the PK profile of the human mAb 3BNC117 after intravenous, subcutaneous, and intraperitoneal injections in four mouse strains: BL6, BCD, RAG2, and NSG mice. Noncompartmental analysis was used to quantify differences in PK between each mouse strain. Strikingly, both BL6 and NSG mice exhibited significantly higher mAb clearance compared to the other two strains. To better understand these differences, we developed a minimally physiological based PK model of mAb PK and estimated model parameters using nonlinear mixed effects modeling. The fitted model parameters illustrated how specific processes change in each strain, including the change in clearance rates over time in BL6 and differences in mAb lymphatic uptake in NSG mice. We then used simple allometric scaling relationships to assess which strains were reasonably predictive of human mAb PK. NSG and BL6 mice were found to be unpredictive of human PK, unlike RAG2 and BCD mice. Overall, these results highlight the importance of selecting an appropriate KO strain for preclinical mAb evaluation and demonstrate that RAG2 and BCD strains are suitable mouse models for investigation of mAb PK. OBJECTIVE
There remains a lack of multi-center nationwide data that examine residents' self-reported level of comfort with obtaining informed consent. Our study sought to acquire these data and discover relationships affecting the subjective experiences of general surgery residents during consent acquisition so that general surgery residency programs can better understand their trainees' needs.
DESIGN
We internally developed a 22-item survey assessing the demographics of general surgery residents who commonly obtain consent, the amount of formal informed consent training provided in residency, and residents' comfort level with the consent process.
SETTING
The study originated from the University of Iowa Health Care Department of Surgery. Our survey was distributed nationally via mass electronic-mailing to recruit participants.
PARTICIPANTS
The survey was iteratively sent to 323 ACGME-accredited general surgery residency programs across the United States via program leadership. Eight weeks were allowed for response accumulation from May to June 2025. There were 140 total survey responses (1.4% response rate) with a likely unrepresentative national sample.
RESULTS
Across all respondent training levels, first-year residents (interns) were identified as the most frequent consenters. Only 14.3% of interns felt "very comfortable" obtaining surgical consent. Although most senior residents reported receiving consent training during their PGY1 year, 73.3% of interns reported not receiving any formal instruction during residency. Interns made up 60% of respondents who reported being "very dissatisfied" with their formal consent training. Furthermore, 64.3% of interns were either very dissatisfied (21.4%) or slightly dissatisfied (42.9%) with consent training. Nearly half of interns reported "usually" obtaining consent for procedures they felt would be more appropriate for senior residents, and 57.1% reported acquiring consent for unfamiliar procedures "about half the time."
CONCLUSIONS
These findings suggest a possible disconnect between expectations and preparation among surgical trainees who are expected to obtain informed consent. This self-reported lack of preparation may represent a lapse in patient safety standards and supports our continued research into the necessity of developing a standardized informed consent curriculum for all general surgery residents with a revamped participant recruitment strategy.

SUMMARY In light of the recent growth in interest and knowledge of post-TB sequelae, there were high levels of engagement during the 3rd International Post-Tuberculosis Symposium held in Stellenbosch, South Africa. This multi-disciplinary symposium aimed to: 1) Advocate for greater global awareness of post-TB sequelae and empower TB-affected communities; 2) Advance knowledge by sharing current evidence and identifying key priorities; 3) Foster collaborations by strengthening research networks and developing concrete plans for research driven advocacy; and 4) Advance the field by establishing areas of consensus around diagnosis, care, and management. Guided by a 14-member Steering Committee, 9 academic working groups came together to develop key content for plenary sessions and facilitated workshops related to: Patient Engagement, Epidemiology and Modelling, Pathogenesis, Post-TB Lung Disease; Cardiovascular and Pulmonary Vascular Disease; Central Nervous System and Musculoskeletal Disease; Paediatrics Economic; Social and Psychological Sequelae; and Advocacy, Policy, and Stakeholder Engagement. Each group outlined progress within their respective fields and defined key priorities to focus discussion. The Symposium further catalysed coordinated action for the post-TB community of patients, advocates, clinicians, and researchers to define a clear path towards improving outcomes, reducing inequities, and ensuring TB survivors receive the care and support they deserve.

Voltage-gated sodium channels are essential for healthy nervous system function. Mutations in voltage-gated sodium channels are associated with a range of seizure conditions. The genetics of seizure conditions are complex and often challenging to study or replicate in animal models. The Drosophila melanogaster gene paralytic (para) is the sole voltage-gated sodium channel gene in flies. The paraShudderer allele causes dominant seizure activity manifest in adult morphology and behavior. In this study we replicated previous findings of paraShudderer hyperexcitability and the ability to suppress this activity with dietary supplementation of an omega-3 fatty acid, alpha-linolenic acid (ALA). Our results support the robustness of the paraShudderer phenotype and the replicability of findings across separate lab environments.

Patients with headache disorders may benefit from a collaborative, interdisciplinary and/or multidisciplinary approach with involvement from multiple physician specialties, therapists, psychologists, and pharmacists. Each member of the team plays a role in headache diagnosis and management, from medication management to patient education to procedural or surgical intervention. Initiation of an interdisciplinary approach for the patient with headache is crucial, especially if the patient fails initial conservative treatment or suffers from other chronic pain states so as to adequately address all aspects of the headache pain. The successful headache management team benefits from understanding each team member's role within the team.

5-Methoxy-N,N-dimethyltryptamine (5-MeO-DMT) is a psychedelic gaining interest as a potential mental health treatment. However, a poorly characterized phenomenon termed reactivation (similar to "flashbacks") is commonly reported with 5-MeO-DMT use and merits investigation. This study examined the prevalence, predictors, and emotional valence of reactivations among Spanish-speaking individuals. Specifically, we investigated whether gender, time planned ahead, intensity of acute effects, and the meaningfulness attributed to the experience predicted reactivation events. We also explored whether decreases in cognitive fusion were associated with reactivations, and whether these variables influenced the emotional valence of those experiences. A cross-sectional survey study was conducted with 90 respondents. Descriptive statistics and logistic regression analyses were employed to examine the aims. Sixty-nine percent of respondents reported reactivations, with 97% indicating the experience was positive or neutral. In the multivariable logistic regression model, longer time planned ahead and decreases in cognitive fusion significantly predicted reactivations. Challenging experiences were associated with a lower likelihood of reactivations being perceived as positive, while psychological insight was linked to a higher likelihood of positive perceptions. Reactivations among this sample appear to be common and predominately perceived as positive or neutral. Their association with cognitive fusion decreases suggests potential clinical relevance worth further investigation.

Weather radars provide surveillance and warning systems that increase public safety and security. The United States is home to one of the most powerful and advanced weather radar networks in the world, which could still be improved in a number of ways, including improved coverage through additional supplemental radars. In October 2016, the University of Louisiana Monroe began operating a polarimetric S-band Doppler weather radar (KULM) in an area without extensive low-level coverage. Here, we evaluate the impact of this installation and operation of the KULM radar on local report climatology, operational meteorology, and casualty impacts of tornadoes over the last ten years—divided into an early (2012–2016) and late (2017–2021) period. Results indicate more than a seven-fold increase in the number of annual tornado reports across the KULM area between the early and late period, a decrease in KULM area false alarm rate, and a nearly unchanged KULM area probability of detection over time, as well as a decline in the KULM area annual casualty rate since the installation and operation of the radar. Holistically, these findings suggest additional investment in radars that provide supplemental low-level coverage have the potential to improve National Weather Service warning activities all while providing significant value to the public that extends beyond the individual tornado hazard.

A 14‐year‐old male presented with toxic megacolon (TM). Stool culture and rectal biopsies ruled out Clostridium difficile infection and Hirschsprung disease, respectively. Anorectal manometry ruled out anal achalasia as rectoanal inhibitory reflex elicited a normal resting pressure. Colonic motility study showed distal colonic dysmotility and was consistent with a history of chronic constipation and encopresis. He initially underwent a diverting loop ileostomy, which ultimately improved his clinical status and resolved his TM without total abdominal colectomy. Diverting loop ileostomy and antegrade colonic enemas for TM should be considered as an alternative to the standard total abdominal colectomy and end ileostomy in the pediatric population.

This article explores how connections brought about through practices of donor conception are changing in the context of mediatisation and, in particular, how digital technologies shape possibilities for donors and recipient families to know one another. We focus on the growing practice of 'informal' donor conception (IDC) in the UK - donor conceptions which are arranged outside of medical institutions and often facilitated via online platforms. Drawing on interviews with 30 (intended) parents via IDC and 19 informal sperm donors, we show how donors and recipients carefully manage, or attempt to manage, the ways in which they are known and knowable to one another, and to a potential donor-conceived person. We identify four ways of knowing, and being known, within participants' accounts: 1) selective sharing of identifying information; 2) (dis)embedding donation connections within wider communities; 3) communicative forms of knowing; and 4) via the creation and management of digital artefacts. Donors and recipients sought to use and limit these different modes of knowing in varied combinations, shaped by their knowledge of (clinical) assisted reproduction practices, changing digital cultures and the legal marginalisation of IDC practices.

Breast cancer (BC) is the most dangerous cancers among women. Therapy resistance to thechemotherapy is the most concerning for the breast cancer management. Protein S-palmitoylation is a post-translational reversible modification of some specific proteins whichhelps them to attach to the cell membrane. Depalmitoylation reverses palmitoylation and is catalyzed byenzymes belonging to the a/b hydrolase family of serine hydrolases called acyl protein thioesterases 1/2(APT 1/2). SCRIB, STAT3, TNFR1, ZDHHC6, and MC1R are the known targets of APT-2 and many of themhave established role in cancer progression. APT-2 inhibitor ML349 inhibited soft agar colony formation, 2D clonogenicity of breast cancer cells. ML349 treatment sensitized breast cancer cells toward Doxorubicin both in vitro and invivo. Both treatment with ML349 and siRNA knock down of APT-2 showed suppression of cell cycle regulators, cancer stem cell markers, YAP/TAZ target genes in breast cancer cells. Patient sample data from TCGA shows that lower expression of SCRIB is correlated with increased Overall and Progression free survival of Breast Invasive Ductal Carcinoma, in doxorubicin treated patients. In conclusion, APT-2 inhibition may suppress cancer stemness and therapy resistance in breast cancer cells.

OBJECTIVES
The underrepresentation of racial and ethnic minorities in clinical research limits the generalizability of findings. This study examined how specific study design elements, such as follow-up requirements, stipends, and research procedures, were associated with participation refusal rates across racial and ethnic groups in a pediatric emergency department setting.
METHODS
This retrospective cohort study analyzed recruitment outcomes from prospective studies conducted between 2012 and 2022 in a pediatric emergency department with approximately 70,000 annual visits. Studies requiring informed consent were included, excluding those targeting specific racial or ethnic groups. Demographic data were abstracted from the electronic health record using standard categories obtained by self-report during ED registration. Logistic regression assessed associations between study design elements and consent likelihood across racial and ethnic subgroups.
RESULTS
Among 14,500 encounters, the median age was 12 years; 14.0% identified as Hispanic, 41.6% as Black, 55.7% as White, and 2.0% as Asian. Recruitment was successful in 71.5% of cases, with a 25.4% refusal rate. Moderate enrollment procedure time (6 to 15 min) were associated with higher consent odds for all groups, while very short (<6 min) or prolonged durations (>30 min) were linked to lower consent odds. High stipends (>$135) were associated with reduced likelihood of consent across all groups. Requirements for biobanking and lab work were more likely to be linked with refusal among Black and Asian participants. Hispanic participants were more likely to consent to medication trials (OR: 1.74, P=0.003). Follow-up requirements were associated with lower consent odds among Hispanic and White participants, with a stronger association for Hispanic participants (OR: 0.39, P=0.002).
CONCLUSIONS
Study design elements were significantly associated with recruitment outcomes, with notable variation across racial and ethnic groups. Transparent communication, culturally responsive engagement, and logistical flexibility may help reduce participation barriers and promote equity in pediatric clinical research.
